NATURE OF WORK : This temporary professional management position has the primary responsibility to provide assistance with the development and annual review of the Quality Review Program (QRP), implement the QRP with assigned Care Management Units (CMUs), and monitor care management activity outcome for assigned CMUs. The role of this position is to and develop, implement, and report program quality improvement/quality assurance initiatives, provide ongoing training and mentoring to CMUs, foster collaborative ventures for training and service opportunities, assure quality services which are cost effective and safeguard consumer choice, and to assist with the development of program policies, procedures and functioning operations for community based care management program older adults, persons with disabilities, and persons with persistent mental illness. This position reports to the Director of Best Practice. REQUIRED QUALIFICATIONS: Strong interpersonal skills and ability to effectively interact with persons from a variety of different backgrounds and experiences in a culturally competent, professional, diplomatic, and courteous manner. Knowledge of special needs and problems affecting older adults, persons with disabilities, and persons with persistent mental illness and the impact on these conditions on the client and their family. Knowledge of the continuum of health and social service providers and resources available in the community including but not limited to health, social services, housing resources and advocacy resources. Possession of a Bachelors in Social Work or related degree and at least ten (10) years experience in care of community-based older adults, persons with disabilities, and persons with persistent mental illness preferably as part of interdisciplinary team is required. One to two years experience in utilization review or program research strongly preferred. Certified or eligible for certification as a social worker preferred. Familiarity with healthcare financing mechanisms such as Family Care Program, Medicare, Medicaid and SSI. DESIRABLE QUALIFICATIONS; 1. Master’s in Social Work preferred. 2. Bilingual skills. 3. Extensive knowledge of PC based software applications, including the Microsoft products line with emphasis on Word and Excel. 4. Possess an abundance of energy necessary to perform effectively in and exciting, fast paced environment. WORK CONDITIONS/ENVIRONMENT: 1. Must be willing to work a flexible schedule with occasional evening and weekend hours. 2. Must be willing to receive crisis calls after normal working hours and take on-call duty.
